Connections Program

Connections is a full-day program, which provides social, physical and cognitive stimulation for adults who may require more assistance during the day. Our participants include seniors and adults with Alzheimer’s disease/dementia, brain injuries, cognitive or physical disabilities or diseases that result in the need for a supported environment. Activities include daily exercise, therapeutic art, music therapy, sensory gardening, modified sports, specialized groups and intergenerational events.

FAQs about the Connections Program

How many days per week can someone attend ADJ?

Your weekly schedule is up to you and your family and what works best. All we ask is that you select how many days per week and what you would like for the days to be. Ex. If you chose to come 3 days per week you could select Monday, Wednesdays, and Fridays as your scheduled days. We do have a limit on the maximum number of people that we can support each day per our state licensure, so specific days will be subject to availability.

Who pays for the program?

We accept a variety of funding options: Medicaid Waiver, DMH Waiver, and the VA. We do offer a Private Pay option, please call us for more details about out daily rates.

Who is a good fit for the Connections program?

We accept individuals who:

  • Are older adults seeking opportunities for connecting and socializing with others.
  • Are able to be engaged and active cognitively and physically throughout the day.
  • Comply with safety expectations, stays with the group, no risk of elopement.
  • Are Independent with toileting and personal care, non-ambulatory individuals must be able to bear their own weight and safely pivot with the use of grab bars.
  • Do not show verbally or physically aggressive or disruptive behaviors.
  • Are able to maintain group level of supports, not requiring more than 1:6 staff-to-participant ratio support.
Do you offer behavior management services?

Our programs offer a maximum of 1:6 staff-to-participant support ratios. We do not have behavior specialists on staff and cannot provide behavior management services.

Do you need to have a diagnosis of dementia or Alzheimer’s?

There is no diagnosis requirement to be eligible for our programs at ADJ. CONNECTIONS utilizes a social-based service model to offer a welcoming and engaging environment for people to connect with others, experience fulfillment and purpose, and keep their minds and bodies active throughout the day. Our programming and activities are designed to specifically appeal to the aging population, while also affording us the flexibility to accommodate the provision of additional supports to participants with diagnoses of Alzheimer’s/Dementia, Parkinson’s or MS, stroke or traumatic brain injury survivors, and those managing a variety of cognitive and/or physical challenges.

Are meals included? What if I have special diet considerations?

Yes! Our participants join us for a delicious and healthy breakfast, lunch, and snack. All of our meals are prepared fresh on-site and served straight from the kosher kitchen and meet USDA Healthy Eating Guidelines. The on-site RN and kitchen staff collaborate to adapt meals for special health needs.

Do you go on outings in the community?

Absolutely! We take our groups to places like the Zoo, Science Center, Parks, Museums, Theaters, Sporting Events, etc., and we routinely go on Reminiscent Joy Rides to a variety of places within our community.

Is exercise part of the programming offered?

We are all too aware of the negative impacts a sedentary lifestyle can have on the body and mind, which is why we prioritize the physical health and wellbeing of our participants and aim to incorporate physical exercise into our daily program plans. Our participants can expect to engage in organized exercise-based activities at least 2 or more times each day. These activities can include yoga, Tai Chi, aerobics, stretching, weights, resistance bands, drumming, dance, and daily walking groups.

What types of activities do you offer for skill building?

During our enrollment process, we work with each participant and their family to identify and establish individualized goals that the participant desires to focus on while attending the program. Maintaining functional independence, socialization, communication, recreation exploration, and physical activity tend to be key focus areas for most of our CONNECTIONS participants. With these goals in mind, we develop recreational activities and offer therapeutic-based programs that are conducive to supporting each participant with maintaining and/or improving their skill sets. Activities include brain boosters, arts & crafts projects, cooking and book clubs, reminiscence activities, intergenerational programs, gardening and nature programs, volunteerism, special events and entertainment, and more. Therapeutic programs include music, speech, pets, gardening, and more.
